Cancellation / No-Show Fee

A minimum of 24 hours notice of cancellation is required. Failure to provide adequate notice of cancellation will result in a $75 fee applied to your account.

Medication & Eyeglass Prescriptions

Please bring a list of your medications and all pairs of glasses that you currently use. If you wear contact lenses, please bring your contact lens information (i.e., prescription, brand, type or packaging).

Alberta Health Card

Please bring your personal Alberta Health Care card to receive coverage for health services included by Alberta Health Care Insurance Plan. Please note that additional fees may apply.

Personal Health Benefits Information

Please be certain to bring any personal health benefits insurance information (i.e., policy, group, etc.) with you. Please note that patients are responsible for managing their personal health insurance plans and should consult their insurance provider regarding plan details (i.e., coverage, renewal dates, ability to direct-bill, etc.) before scheduling an appointment. Please note that additional fees may apply.
